Comprehensive Client Assessment and Reporting

In one recent case, we demonstrated our thorough approach to client assessment:

  • Initial Assessment: A client initially received 4 hours of weekly NDIS support. However, we recognised the need for a more extensive evaluation and proposed a 24/7 observation over a week to gain a full picture of the client’s needs.

  • Service Provision and Reporting: After obtaining the family’s agreement and signing a service agreement, we provided the necessary care and conducted an in-depth assessment over a 7 day period using a 24/7 model of care. This involved:

    • Summarising the client’s patterns and daily needs.

    • Recommending appropriate programs and allied health practitioner involvement.

    • Compiling a comprehensive report detailing hourly care needs and long-term support requirements.

Our findings were crucial in securing preliminary funding for adequate services, ensuring the client received the necessary support for independent living.

Meet Klade Thomas: Clinical Expertise You Can Trust

Klade Thomas brings over 20 years of clinical nursing experience to his role as National Manager – Clinical, Quality & Safeguards at ONCALL SACARE. His extensive qualifications and diverse experience, ranging from intensive care units to community nursing and international aid work, make him a trusted expert in supporting legal cases.

Klade’s journey in clinical care began at the Royal Adelaide Hospital, where he served in cardiac and intensive care units. His career later expanded to include community nursing with the Royal District Nursing Services, where he provided care for individuals with severe spinal cord and brain injuries, as well as those with psychosocial disabilities.

Klade’s expertise also extends internationally, having provided Ebola nursing care in Liberia and consulted on healthcare projects in conflict-affected areas in Nigeria. Upon his return to Australia, he contributed to establishing new health clinics and managed various supported accommodation facilities within SACARE.

Since joining SACARE in 2017, Klade has overseen all clinical aspects of complex care and clinical governance. His role now includes managing allied health services, behavior support services, and the THRIVE program across South Australia, Victoria, and Queensland. His dedication to clinical excellence and comprehensive care makes him an invaluable resource for legal professionals seeking expert support.
